    This application must be signed by all adults who will occupy the apartment/duplex before it can be considered by Landlord. Acceptance of this application, and any monies deposited herewith is not binding upon Landlord until approved by landlord in writing. If approved, monies deposited with this application will be held as a reservation deposit to be either returned to applicant or credited toward any deposit, which may be required of an applicant at the time, a rental agreement is secured. If approved and the rental unit is held for applicant for more than three days then the applicant withdraws the application, all monies deposited shall be forfeited to landlord. NON REFUNDABLE PROCESSING FEE: $45.00 & NON-REFUNDABLE ADMINISTRATIVE FEE $350.00.

    In compliance with the FAIR CREDIT REPORTING ACT this notice is to inform you that processing of this application includes but is not limited to making any inquiries deemed necessary to verify the accuracy of the information herein, including procuring consumer reports from consumer credit reporting agencies and obtaining credit information from other credit institutions. The undersigned agrees this application and any information reports will remain the property of Willow Chase/ Ridgeway. 

    I hereby grant this property the right to process this application for the purpose of obtaining a Rental/Lease Agreement with this property. Additionally, I authorize all corporations, companies, landlords, law enforcement agencies, academic institutions, and current and former employers to release information they may have about me and release them from any liability and responsibliity from doing so. An electronic, faxed, or other copy of this authorization shall be as valid as the original.

    The deposit and administrative fee shall be refunded if Landlord rejects rental application.  Applicant has three (3) days beginning the day he/she is notified of their approval verbally to cancel this agreement.  If canceled by applicant after three (3) days, the entire deposit and administrative fee ($350.00) shall be forfeited as liquidated damages, including but not limited to accrued rent. Duplex/Apartment availability can be changed at any time by owner.  Applicant will be assigned another duplex/Villa that is the same layout, but different location due upon availability. We reserve the right to only hold a duplex only 15 days, before address will be changed. 


    *If you have put down a deposit on a home sight unseen the 3-days will still apply. If once you have seen the home, you do not wish to move in and it has been past your 3-day grace period, your deposit will still be forfeited as stated above.

    Landlord agrees that subject to the conditions listed below, the refundable portion of the deposit will be returned 


    Resident agrees that this deposit may not be applied as rent, and the full monthly rental will be paid on or before the first day of every month including the last month of occupancy.


    Release of the Security Deposit is subject to the following provisions:

    1.         Full term of lease has expired

    2.         Sixty days’ written notice prior to the first of the month was given prior to moving out of apartment

    3.         No damages to property beyond normal wear and tear

    4.         Entire duplex/villa including range, refrigerator, bathroom(s), closets, and cabinets are clean

    5.         No stickers or scratches on walls

    6.         No unpaid charges or delinquent rent

    7.         All keys returned

    8.         All debris and trash are discarded in proper rubbish containers

    9.         Forwarding address is left with management

    10.        Cleaning expectations have been fulfilled & returned to the original state at move in.

    11.        No prior charges on left open on the account.


    If the provisions above are complied with the security deposit will be refunded by check, mailed to the forwarding address, made payable to Resident within thirty (30) days after the duplex/Villa has been vacated or notification of cancellation is received.


    Resident hereby gives Landlord the right to commingle security deposit funds with the properties operating account.

    Welcome to our community.  Before you apply to rent a villa, please review our rental application approval criteria.  The following information is offered so that applicants will have a detailed statement of rental qualifying policies.  Although we have attempted to make this document easy to read and understand, by its nature as a statement of policy, it includes formal language and legal terms. Any questions about the information in this document may be directed to any member of our management team.

    Definitions

    The term “applicant(s)” in these criteria means the person(s) that will be signing the lease as a “resident”.  The term “occupants) in these criteria means the person(s) that are authorized occupants under the lease.  Please also note that these are our current rental criteria; nothing contained in these requirements shall constitute a guarantee or representation by us that all current residents and occupants have met these requirements. There may be occupants that resided on the premises prior to these requirements going into effect.  Additionally, our ability to verify whether these requirements have been met is limited to the information we receive from the various credit, criminal and evictions reporting services used.

    Non-Discrimination

    We will not discriminate against any person in the rental of a villa because of race, color, religion, national origin, sex, age, familial status, sexual orientation, or mental or physical handicap.

    Confidentiality

    We maintain a strict policy of confidentiality and privacy for our applicants and residents.  We do not discuss information on applications with anyone other than the applicant.  In addition, we do not discuss individual credit reports with an applicant.  If you would like to discuss or dispute anything in your credit report, you will need to contact the credit reporting agency that provided the report. Upon your request, we will provide you the name and address of that reporting agency.

    Community Policies: Maximum Number of Occupants – Prohibited Pets – Parking Restrictions.

    * 3 Bedroom= maximum of 6 occupants.  2 Bedroom= maximum of 4 occupants.  Exceptions may be made to this policy based on the familial status of occupants.  All occupants 18 years of age and older will be considered as residents under the lease agreement and will be asked to sign the lease as a responsible resident.  A rental application is to be completed by occupant(s) 18 years of age and older, without omission of falsification of information.

    * No more than two vehicles are permitted per villa

    * Aggressive breeds of dogs and exotic, feral or wild animals are prohibited. Dogs only up to 35 lbs. Choice Management LLC complies with State & local regulations as well as Federal ADA regulations. “Service Animal” means any animal trained to do work or perform tasks for the benefit of an individual with a disability. Medical documentation required to support tenant request for exemption. Pets providing emotional support, wellbeing, comfort or companionship are not recognized as service animals under the ADA regulations. 

    Age/Identity

    Verification Applicants must be at least 18 years of age. A government issued photo ID is required of all applicants and guarantors who wish to tour a villa or model.  Applicants from foreign countries who have no social security number or citizenship must have a proper and current US Visa, I-9 documentation, and meet other criteria for consideration

    Criminal History

    Criminal checks will be conducted.  A felony conviction, any felony or misdemeanor conviction for a crime against a person, or incarceration for any offense will not be accepted.  “Conviction” includes but is not limited to a deferred judgment; a withheld adjudication; a plea of nolo contender; a guilty plea; or a plea bargain to any lesser charge, including to a misdemeanor.  “Incarceration” means being jailed pursuant to a conviction, not a mere arrest. Conviction of any sexual offense will not be accepted.  Convictions for any terrorism related charges will not be accepted.

    Applicants must provide their last 2 consecutive pay stubs or the most recent W-2 or previous taxes, and/or allow Choice Management LLC to contact employer to verify employment.  New employment may be verified with an intent to hire letter.

    The card above will be charged $45.00 as an application fee. The card above will also be charged $1.35 for the services of Property Matrix online applications. I agree to the terms that these charges are NON-REFUNDABLE.

    I(We) do hereby authorize Choice Management LLC hereafter named COMPANY, to initiate to (my/our) account indicated at the depository financial institution named above, hereafter named FINANCIAL INSTITUTION. I further authorize COMPANY to initiate an adjusting or correcting entry as necessary. Finally, should any such debit(s) be returned as NSF or Uncollected Funds, I(we) authorize the COMPANY to collect such debit(s) electronically and to subsequently collect a Returned Item Fee of $25.00 (or the maximum allowed by state law, whichever is greater) per item, electronically from the same account identified above.

    Property Matrix is an independent property management software company. By using this service, you are contracting with Property Matrix to process this transaction. The Portal is an amount charged by Property Matrix to you the customer for payment access through the Property Matrix online applications. The Portal Fee is nonrefundable.
